Care - the link between
hospital and home.
Lake Taylor specializes in a number of services that require treatment
when a patient is too well to benefit from a hospital stay, yet
still requires the attention of healthcare professionals. A stay
at an acute care hospital can become prohibitively expensive. Lake
Taylor Transitional Care Hospital has become the choice of many
case managers throughout Southeastern Virginia. We work closely
with patients and families to orchestrate care plans that result
in the fastest recovery and best quality of life possible.
From the hospital, through
rehabilitation and recovery, to the return home - the ultimate goal
is to return patients to a productive, healthy lifestyle.
Admissions - A welcome experience.
One thing never changes at Lake Taylor Transitional Care Hospital.
It's our commitment to providing a level of care that facilitates
the highest quality of life for all of our patients. Our doors are
always open to new patients and old friends. You'll notice, from
the moment you enter, the warm touches that have made Lake Taylor
an up-and-coming force in healthcare in Southeastern Virginia.
Planning
for discharge - As soon as you walk in the door.
What sets Lake Taylor apart from many transitional care facilities?
Our successful discharge rates and length-of-stay record. The averages
shown below are well within and often lower than national averages
thanks to the combined determination of the patients, case managers
and healthcare professionals who work collectively for a successful
rehabilitation and recovery. And in some cases our success rates
go far beyond the national average. Our ventilator weaning program
has become a signature program for us because of our multi-dimensional
approach to respiratory rehabilitation. 79% of our respiratory ventilation
patients are discharged after rehab. That's nearly triple the national
average of 29%.
