LPN : Charge Nurse/ Skilled Care
As a state-licensed facility, empowered by a 125-year heritage of serving the citizens of Norfolk and Hampton Roads, Lake Taylor provides a continuum of patient care ranging from long-term acute care hospital services to skilled and intermediate nursing care.
The Charge Nurse works under the direct supervision of the Nurse Manager. The Charge Nurse assesses,plans, implements, and evaluates individual patient care on a nursing unit according to unit policies and procedures. Performs comprehensive assessments of patients. Updates individualized care plans. Demonstrates competence in the role of leader. Oversees the delegation of tasks to Certified Nurse Aides and monitors for compliance with policies and procedures.
Required Experience: 1-2 years facility-based experience in long term care, skilled nursing.
* VA state-issued (current and unencumbered) license to practice as a practical nurse.

CHARGE NURSE: Acute Care
Description :
Lake Taylor Transitional Care Hospital is looking for exceptional nurses to join its team! The RN Charge Nurse will supervise the clinical team of CNAs, RNs, and LPNs while assisting in the development and implementation of patient care plans and interdisciplinary team reports. The Charge Nurse will assist the clinical nurse team in identifying goals and approaches to patient centered care and help lead the acute care unit toward a goal of nursing excellence.

UNIT MANAGER/ LONG TERM CARE
DESCRIPTION:
Works under the direct supervision of the Director of Nursing. The Nurse Manager, for this Acute Care Nursing Unit, manages and is responsible for his/her Unit on a 24 hour basis.
- Evaluates job performance, resolves employees issues, and administers discipline when necessary for all employees of his/her assigned units.
- Responsible for the quality of work, the productivity of the group, the morale of the group, the expense for the area, and employee adherence to hospital and departmental policies, procedures and guidelines.
- The Nurse Manager will be expected to provide hands on patient care on his/her units to participate in QI activities and perform other duties as assigned.
REQUIREMENTS:
- RN degree from an accredited school of nursing. BSN preferred.
- Five years of staff experience, with at least two years of supervisory experience.

- Additionally, for the group that he/she supervises, the Head Nurse is responsible for the quality of work, the productivity of the group, the morale of the work group, the expense for the area, the working relationship with physicians, and employee adherence to hospital and departmental policies, procedures and guidelines.
- The Head Nurse will be expected to provide hands-on patient/resident care on his/her unit.

Cook (Full-time)
Description
- Read menus, recipes, and review all necessary steps in meal preparation with the Food Services Manager.
- Prepare all necessary menu items, determine quantity of food needed and follow recipes.
- Serve meals using portions listed on menu; use proper food preparation and handling techniques.
- Prepare substitute items to meet resident’s individual needs.
- Cover, date and label all leftovers and store properly.
- Follow established security routine; use proper infection control techniques and protective equipment; use proper safety techniques.
Requirements
-
- The Cook must be knowledgeable about safe food preparation and handling techniques, nutritional standards/applications and be skilled in bulk food preparation.
- Should be accurate, concise and detail-oriented and able to work cooperatively as a member of a team.
- Six months of experience in large quantity food preparation; experience with therapeutic diet preparation is preferred.
- The Cook must be knowledgeable about safe food preparation and handling techniques, nutritional standards/applications and be skilled in bulk food preparation.

Maintenance Mechanic
Description:
Performs a wide variety of electrical, mechanical, and installation duties, with an emphasis on plumbing. Includes using technical training and acquired skills as well as prints, schematics, technical manuals and departmental procedures to maintain physical plant, equipment and systems. Duties include:
* Plumbing
* Carpentry
* Painting
* Other skills required to maintain a safe and clean facility.
Education/Special Skills Required:
- Must be able to work on a team of maintenance techs and other specialists, as well as be able to work on your own.
- One or more years of comparable experience is required.
- A valid and clean driver’s license is required, as this person will be required to drive a company vehicle.
- High school graduate or GED.
- Schedule: 7:30AM-4PM (Mon.-Fri.)
- Occasional weekends.
Internal Case Manager
Case Management services are generally focused on members who fall into one or more high risk or high cost groups and require significant clinical judgment, independent analysis, critical-thinking, detailed knowledge of departmental procedures, clinical guidelines, community resources, contracting and community standards of care. Case Management includes assessment, coordination, planning, monitoring and evaluation of multiple environments. Must be licensed RN/LPN in the Commonwealth of Virginia.
1. Coordinates the integration of social services/case management functions into the patient care, discharge and home planning processes with other departments, external service organizations, agencies and healthcare facilities.
2. Conducts concurrent medical record review using specific indicators and criteria as approved by medical staff.
3. Promotes effective and efficient utilization of clinical resources and mobilizes resources to assist in achieving desired clinical outcomes within specific time frame.
4. Conducts review for appropriate utilization of services from admission through discharge. Evaluates patient satisfaction and quality of care provided.
5. Communicates with physicians at regular intervals throughout hospitalization and develops an effective working relationship. Assists physicians in maintaining appropriate cost, case and desired patient outcomes.
6. Responsible for the coordination of health care decisions by using a systematic approach to assure treatment plans that improve quality and outcomes.
7. Working with the complex care needs.
REQUIREMENTS:
- Licensed RN or LPN with 2 years case management experience.
- Experience in insurance-based case management REQUIRED. Previous computer experience, particularly with the use of Microsoft Office is required.

Thorough knowledge of case management principles, healthcare management and reimbursement.

Knowledge of CMS guidelines and experience working with payor specific guidelines and contractual rules preferred.

Customer service abilities including effective listening skills.

Environmental Services/Patient Services Aide
Description:
Will assist in keeping assigned patient, office, lobby, halls, stairways, restrooms, and other areas and equipment, clean, aseptic, neat and safe.
