MDS Coordinator

Works under the supervision of the Director of Nursing, assists in implementing and monitoring OBRA related programs. Demonstrates knowledge of the RAI system. Current knowledge of HCFA/OBRA regulations as related to RAI process, PPS and Electronic transmission. Maintains current policies, procedure and protocol related to RAI process inclusive of care plans, meetings, and electronic transmission. Ensures consistent interpretation of MDS current RAI manual instruction guide by all interdisciplinary team members. Communicates changes/updates to the team. Accurate scheduling of all types of MDS (PPS, Admission, annual, quarterly, significant change, significant corrections). Coordinates the on-time completion of initial, comprehensive, change of condition, discharge and quarterly assessments, CAA’s and care plans.
* Prior experience as an MDS coordinator preferred.
* RN with current, active license in the Commonwealth of Virginia

Unit Secretary
Description
- Performs a variety of non-clinical and ancillary services.
- Clerical support and related unit support functions.
- Facilitating effective communication with inter/intradepartmental personnel to maintain an efficient, and organized nursing station.
- Writing and printing skills are legible.
- Experience in medical terminology, and proficient in computers and typing.
- Assist in maintaining medical record accuracy and integrity.
- Able to perform general receptionist duties.
- Monday through Friday schedule.
- 7:00am – 3:30pm
Requirements/Education/Licensure
- Thorough and working knowledge of medical terminology.
- High school diploma or equivalent
- Minimum 1 year experience in hospital (acute care) or pharmacy setting.

DESCRIPTION
- Secures required signatures on documents; explains hospital regulations and policies; verifies insurance coverage; provides the Medical Record’s office with all necessary paperwork; and obtains a Medical Records Number.
- Verifies receipt of required paperwork, and arranges for the receipt of missing paperwork. Types and distributes care plan for newly admitted Nursing Facility residents.
- Secures the department director’s signature on all appropriate financial documents.
- Prepares and distributes an admissions packet to all appropriate parties within the hospital.
- Provides information concerning facility services via telephone, mailing of literature, and Hospital tours to prospective patients, and/or representatives, as required.
REQUIREMENTS

High School graduate.

Working knowledge of Medicare, Medicaid and other insurance payor sources.

Exceptional communications and customer service skills.
- Strong computer skills required.
CERTIFIED WOUND CARE NURSE (LPN)
- LPN with current VA nursing license in good standing.
- Recent experience as a wound care nurse in a healthcare environment
- Excellent assessment and clinical skills
- Should be familiar with wound care procedures including assessments, measurements, treatment protocols, documentation, etc.
- Must be able to work in collaboration with the nursing team
- Should be able to communicate wound progress to the Charge Nurse and attending physician as needed
- Solid work history
- Current VA LPN license required
- Previous experience required
- Current BCLS certification
- Minimum 1-2 years acute care or Skilled Nursing care experience
- 2 years Advanced Wound Care nursing experience.
- Wound Care certification preferred.
RN’s / LPN’s / CNA’s
- 12-hour shifts (7AM-7PM or 7PM-7AM)
- Minimum 1-year nursing experience required.
- Must be licensed in the Commonwealth of Virginia
- Experience in a hospital setting preferred
- Vent/Trach experience desired but not required
- Sign-on bonuses!! (for RN’s & LPN’s)
- CNA’s starting hourly rate $14.00

CHARGE NURSE: Acute Care
Description :
Lake Taylor Transitional Care Hospital is looking for exceptional nurses to join its team! The RN Charge Nurse will supervise the clinical team of CNAs, RNs, and LPNs while assisting in the development and implementation of patient care plans and interdisciplinary team reports. The Charge Nurse will assist the clinical nurse team in identifying goals and approaches to patient centered care and help lead the acute care unit toward a goal of nursing excellence.
